What is CZ citation style for compound family name

What is CZ citation style for compound family name - compound surname - containing more that one preposition or particle?

For instance, my middle name is of a compound family nature: "Gomes de Paiva" and my father's family is another compound one "de Faria" - note that there exist other families with most similar names with or without a preposition or particle "de Faria" and "Faria" (not to mention the respective plurals).

It results that the proper citation could be:

DE FARIA, RICARDO Gomes de Paiva or

DE FARIA, R. G. de P.

See: Essays of an Information Scientist, Vol:5, p.26-30, 1981-82 Current Contents, #7, p.5-9, February 16, 1981 [[1]]

"Table 1: Particles commonly associated with compound surnames. These particles will now be combined with an author’s last name, and the sum will become the heading underwhich that name will appear in the Citation and Source Indexes of the Science Citation Index."

- yet U.S. Visa Form DS-156 [[2]] produced an output for my name as “FARIA, RICARDO Gomes de Paiva de”.

In an opposite manner, MERCOSUL Passport official syntax for the above is Surname, Given name:

Gomes de Paiva DE FARIA, RICARDO
