Where should contrast go?

There is a major new consensus statement, but I'm not sure where to put it. While it's titled "echocardiography", it also includes peripheral imaging. Do we need something inbetween ultrasonography and this level, to deal with flow imaging generally, cardiac or not? Is there much Doppler that doesn't deal with fluid flow? Howard C.
